Schools may use a purchase order. You must retain an additional $200 for return shipping.
The person signing this Costume Rental Agreement, known hereafter as the “applicant,” has read and agrees to the following:
- The applicant hereby agrees and undertakes to save and hold harmless the School District of North Crawford from any and all claims for damages, personal or otherwise, that may arise out of the use of the rental goods, whether by a member of this organization or by other person using or enjoying said goods and without regard to whether the damage, personal, or otherwise, is brought about or caused by negligence whether on the part of the representative, organization, the school district or all three.
- The applicant will be responsible with goods and will forfeit security deposit if goods are damaged. Reasonable wear and tear is okay; contact us about major damage.
- Goods should be returned in good condition by deadline at applicant’s expense.
- Applicant will insure the goods for $2000 for shipping
- Dry cleaning cost ($100) will be paid by applicant, but will be done upon return by North Crawford Playhouse.
- Applicant will not permanently alter garments; temporary alterations are okay.
- Applicant will return items in each box on the inventory list.
