Under Wisconsin Statute ยง 252.04, students admitted to a Wisconsin public school must provide written proof of required immunizations or documentation that they are in the process of completing the required vaccine schedule. This documentation must be submitted within 30 school days of enrollment. If proof of immunization is not provided, the school must notify the parent or guardian and may ultimately exclude the student from school until compliance is met. The law also allows exemptions if a parent or guardian submits a written waiver for medical, religious, or personal conviction reasons. These requirements are intended to prevent the spread of vaccine-preventable diseases within schools.
